Four people died while two had to be rushed to hospital when the Corsa bakkie they were traveling in overturned on the N2 near Tsolo Junction on Saturday morning.
Provincial department of health spokesman Sizwe Kupelo said the four people died on the scene and two survivors were rushed to hospital.
Kupelo could not confirm the direction the car was travelling towards.
"It overturned, at this moment it is difficult to tell which side it was coming from," he said.
It was not immediately clear what caused the accident.
