Dear prospective parents

In light of the Covid-19 pandemic and the fact that we would like to lessen the traffic in the office for everyone’s safety, we will be making our application forms for 2021 available in electronic format.

You may e-mail office@dalejunior.co.za to request a set of forms. 

For our electronic register, in your letter of request, we will require:

1. Biological parent’s full name and surname
2. Biological parent’s I.D. number
3. The name of the learner
4. The learner’s I.D. number
5. Grade of the learner
6. Biological parent’s current contact cell number

We will e-mail a set of forms to you which you may return via e-mail.

Please note: Should your son be accepted at Dale Junior in 2021, we will require all of your attached documents in ORIGINAL FORMAT.
